Under the direction of the department manager, the Medical Assistant will be assisting the team in referral management for High-Risk patients and helping to assist with patient needs such as completing preliminary health assessments. Requirements Responsibilities/Day in the Life: Receive new patient referrals from clients Create charts for all new patients Input medical information in new chart Help Care Coordinators/Intake Coordinators with patient needs as applicable Foster relationships with their team to ensure the CareHarmony culture is positive and supportive. May be responsible for completing telephonic health assessments or chronic care management (CCM) calls with patients Other duties as applies Required Skills/Abilities: Great attention to detail Excellent organizational skills Excellent verbal and communication skills Excellent professional presence when dealing with colleagues Excellent time management skills with a proven ability to meet critical deadlines. Fundamental Skills with Microsoft Office Suite Required Education and Experience: Medical Assistant certificate must be active and in good standing Physical Requirements This position is sedentary and will require sitting for long periods of time This position will require the ability to speak clearly and listen attentively, often by telephone, for an extended period of time The position will require the ability to understand, process, and take thorough notes in real-time on telephone conversations Pay and Benefits: $16/hr - $19/hr based on job duties being performed Health Benefits (core medical, dental, vision) Paid Holidays Paid Time Off (PTO) Sick Time Off (STO) Work from Home CareHarmony LLC
