Our growing home health agency is looking for part?time, per diem RN's to case manage patients in the Western Suburbs and Chicagoland area (North and South sides). Including: the counties of Cook, DuPage, Kendall, Will, Grundy McHenry, Winnebago, Kane and Lake. Previous home health experience preferred. Candidate must be willing to travel and have previous EMR charting experience. This is a part?time, fee for visit position Monday thru Friday with schedule flexibility. Preferred hours of work are 8:30am-4:30pm. Please include resume for consideration.
