*JOB DETAILS*
* $12,000 Sign On Bonus, paid first pay period following 90 days of employment with OIH
* Salary $110,000-$115,000 based on Experience and FTE
* Flexible with hours and scheduling
* Health, Vision, Dental Benefits paid at 50% of Base Plan offered by OIH, eligible after 60 days of employment
* 401K Benefit: OIH matches 100% of your contributions up to 3% of your compensation, plus OIH matches 50% on your deferrals that are greater than 3%, but less than or equal to 5% of your compensation (Eligibility Requirements: 1yr employment, 1000hrs worked, at least 21yrs of age)

* State Medical License and DEA 100% reimbursed, completing 90 days of employment

* 8 Holidays Per Year:

* PTO 160 Hours Per Year:
* Vacation PTO accrues based on time worked, with a maximum annual accrual of 80 hours per FTE 1.0, available for use after 90 days of employment
* Frontloaded PTO is uploaded based on time worked, with a maximum annual eligibility of 80 hours per FTE 1.0, available for use after 90 days of employment
* Protected Sick Time accrues 1 hour for every 30 hours worked, max use of 40 per year, available immediately as time accrues after first day of employment

Oregon Integrated Health is an Integrated Primary Care Medical Home that is hiring for a Full Time/Part Time Naturopathic Doctor for our Rural Health Candidate Clinic in Florence Oregon.
Our Florence clinic sees a demographic of Medicaid and Medicare individuals that are in need for our expansion with community resources that a Rural Health designation will provide.
Our Integrated Medical Home in Florence currently has a team of Family Nurse Practitioner, Licensed Professional Counselor, Medical Assistants, a Community Health Worker, Front Desk and our Senior Director of Clinical Operations. Our patients have access to Quest Laboratories directly in our facility. We are also expanding our space to initiate the Vaccine for Children program.
Oregon Integrated Health recently was awarded a grant for our Mobile Medical Van that will be providing home care visits, mobile telemedicine visits and medical transportation for home bound patients.
We utilize telemedicine visits for patients that are at home as well as telehealth visits – when patients may not have access to data or smart phones patients come to our Florence facility and utilize our technology to connect with any of our providers across our 4 locations! Our additional locations in Eugene, Salem and Portland are comprised of Family Nurse Practitioners, Naturopathic Physicians, Psychiatric Mental Health Nurse Practitioners, Counselors, Chiropractors, Acupuncture, Physical Therapy, Massage Therapy and oral health with an Expanded Practice Dental Hygienist.
*JOB DUTIES AND RESPONSIBILITIES*:
- Conduct patient assessments and gather medical history
- Perform physical examinations and vital sign measurements
- Provide naturopathic treatments and therapies to patients
- Develop personalized treatment plans based on individual needs
- Educate patients on lifestyle changes, nutrition, and natural remedies
- Administer IV infusions and insert IV lines as needed
- Perform venipuncture for laboratory testing
- Provide care for pediatric patients
- Collaborate with other healthcare professionals to ensure comprehensive patient care
*Qualifications*:
- Degree in Naturopathic Medicine from an accredited institution
* State of Oregon license
* Full prescription authority
* Drug Enforcement Agency Certificate
* National Provider Identifier (NPI)
