Job Title: Therapist - Physical Therapist
Location: International Falls, MN
Schedule / Job Type: Full-time, Day shifts, Monday through Friday, 8:00 AM – 4:30 PM

Key Responsibilities:

• Provide home health, hospice, and long-term care therapy services within a shared caseload.
• Manage approximately five patient cases daily, including covering skilled nursing facilities as required.
• Supervise PTAs as needed, with no onsite supervisor present.
• Document patient progress remotely using the designated software platform.
• Travel within an 80-mile radius, with mileage reimbursed at government rates; drive time is billable.
• Adhere to productivity expectations that may vary due to shared caseloads.

Required Qualifications:

• Valid license to practice as a Physical Therapist in Minnesota.
• Home health care experience is required; new graduates with clinical home health experience are considered.
• Ability to supervise Physical Therapist Assistants (PTAs).
• Reliable transportation with current vehicle insurance, and willingness to travel within the specified radius.
• Excellent communication skills and ability to work independently in a remote setting.

Preferred Experience / Skills:

• Previous experience working in home care, hospice, or long-term care environments.
• Familiarity with remote documentation software used in home health settings.
